The **Medical Assistant** will provide clinical medical assistance to the comprehensive medical team serving members at a PH Service Center. The **Medical Assistant** reports to the Service Center Supervisor with primary role to assist and support the Nurse Practitioners at the Service Center with medical assessment and documentation of comprehensive physical examinations on Peoples Health Members.
+ Improve health care quality and increase likelihood of desired health outcomes
+ Collaborates with other clinicians to provide health promotion, health maintenance and sick - care services in an effort to improve our plan member's quality of life and to facilitate optimal health outcomes through cost-effective healthcare.
+ Escorts patient to examination room, obtains and documents vital signs, weight, height, and other information as required.
+ Ensures member satisfaction by assisting with patient / scheduling flow to limit unnecessarily wait times.
+ Prepares and documents in electronic medical record (EHR) to include: medical, surgical, immunization and social history, medication list, risk scores and HEDIS screening needs.
+ Provide quality chart preparation as evidenced by monthly chart auditing within compliance standards.
+ Prepares and assists with the Nurse Practitioner during comprehensive exam, completion of lab, referrals, diagnostic and other clinical appointment.
+ Maintains a clean clinical environment and ensures accurate supply levels in exam rooms as needed. Advises manager when supplies are needed.
+ Assists with coverage of front desk and with scheduling calls to members.
+ Documents telephone encounters for all member conversations.
+ At the authorization by a Nurse Practitioner, and within the scope of the medical assistant's license and regulations, may administer injection(s) as delegated.
+ Prevent hospital readmission
+ Working as a member of the interdisciplinary care team, identifies and coordinates health care and benefit options for the member and/or care giver to overcome potential barriers from discharge to an alternate level of care.
+ Monitors performance against assigned member's gaps in HEDIS and Star measures through weekly report analysis.
+ Implement activities to improve patient safety and reduce medical errors
+ Maintains competency in clinical practice and meets and maintains requirements for certification.
+ Participates in quality improvement activities and monitoring of health outcomes for patients.
+ Ensures collection and documentation of HEDIS/Quality measures and risk adjustment diagnoses as appropriate.
+ Ensures proper documentation in medical record and electronic systems to ensure appropriate coding.
+ Provide and support wellness and health promotion activities
+ Documents medication review for member seen in the Service Center.
+ Support health information technology (HIT) in health care quality improvement
+ Works in coordination company-based security health information systems technology to ensure appropriate documentation of the member's care coordination and record.
+ Participates in ongoing training to ensure the records are accurate and secure.
+ Follows corporate instruction based on Federal and State guidance related to health information documentation and security.
+ High School Diploma / GED or higher
+ Current Medical Assistant Certification from an accredited licensed program
+ Ambulatory care / clinic experience
+ Experience in electronic medical records
+ Basic computer and math skills to perform accurate and efficient data
+ Ability to travel locally up to 10% of the time to one of the other 7 service centers, as needed
+ Current BCLS (Basic Cardiac Life Support) certification
+ Prompt and dependable
+ Sound clinical judgement, assessment, planning and care coordination skills

**Physical Requirements and Work Environment:**
+ The work involves considerable physical exertion, such as frequent lifting of patients and equipment, bending and stooping, and walking.
+ The work environment involves moderate risks or discomforts, which require special safety precautions, e.g., working with risk of exposure to contagious disease or infection, or working with emotionally, disturbed patients.
+ Precautions are routine for nearly all situations.
+ The medical assistant may be required to use protective clothing or gear to continuously maintain universal precautions.
+ May be asked to travel (i.e. using personal vehicle) as needed.
